Reverse osmosis water is safe for most people with a balanced diet, but long-term exclusive consumption without remineralization may create health risks due to removed calcium and magnesium.
You already know RO systems strip out lead, arsenic, and PFAS. What nobody tells you is they also strip out the minerals your body uses every day. Whether RO water ends up healthy for you comes down to one question: are you replacing what the filter removes?
What RO Removes — The Good And The Bad
Reverse osmosis removes up to 99% of dangerous contaminants: lead, arsenic, nitrates, PFAS, perchlorate, hexavalent chromium, and microplastics. For water containing these, RO is the right tool. But the same process removes 92–99% of essential minerals — magnesium, calcium, iron, and manganese — leaving water with a TDS (total dissolved solids) level often below 50 mg/L.
That’s the core trade-off. You get the cleanest possible water and lose the minerals that make water more than just H₂O.
Can Drinking RO Water Cause Health Problems?
In healthy adults with a normal diet, the short answer is no — you get your minerals from food, not water. No clinically significant mineral deficiency has been shown in this group from drinking RO water alone.
The longer-term picture is different. Chronic consumption of low-mineral water has been linked in research to increased cardiovascular risk (from magnesium deficiency), reduced bone mineral density, and accelerated tooth decay because low-mineral water doesn’t support tooth remineralization. The WHO has noted that water with TDS below 100 mg/L may have an adverse influence on the human organism.
RO water is also mildly acidic, with a pH typically between 5.5 and 6.5. The body buffers this easily — there’s no strong evidence tying RO’s pH to health problems in healthy people.
Who Actually Needs RO?
For those people, RO is overkill — a good activated carbon filter handles most concerns without stripping minerals. RO becomes necessary when your water contains nitrates, arsenic, or PFAS, which standard filters don’t remove.
RO systems also waste roughly three times the water they produce and need a supply-line pressure of 40–80 psi to function.
Making RO Water Healthy To Drink
If you already have an RO system or need one for contaminant reasons, three steps fix the mineral problem:
- Add a remineralization cartridge — these restore calcium and magnesium, targeting a TDS level of 200–250 mg/L. This is the single most effective fix.
- Keep a balanced diet — mineral-rich foods (leafy greens, dairy, nuts, legumes) cover what the water lacks.
- Replace membranes and filters on schedule — old filters breed bacteria, which defeats the whole purpose.
The recommendation from research is straightforward: aim for a minimum of 10 mg/L magnesium, 30 mg/L calcium, and 100 mg/L total dissolved salts. A remineralization cartridge on an RO system hits these numbers easily.

Is RO water bad for your teeth?
Low-mineral water doesn’t support tooth remineralization the way mineralized water does. Over time, this may contribute to cavity development, especially if your diet is high in sugar or acid. A remineralization filter or fluoridated toothpaste helps offset the risk.
Does RO water leach minerals from your body?
No. The body doesn’t work that way — water doesn’t pull minerals out of your tissues. The concern is simply that RO water provides fewer minerals than mineral-rich water, so your dietary intake matters more.
Should I switch from RO to a carbon filter?
Depends on your water. If your tap water is within EPA limits for contaminants, a carbon filter removes chlorine and improves taste without stripping minerals — making it the simpler choice. If your water contains nitrates, arsenic, or PFAS, RO remains the safest option, and remineralization fixes the downside.
